Visa Debit Card Dispute
This form is for use in disputes involving incorrectly posted Apple Visa® Debit Card transactions. Though the card holder may have approved the transaction at the point-of-sale, once posted, the occurrence either: (1) posts as a different dollar amount; (2) appears on and is charged to the account more than once; or (3) is not credited to the card holder’s account after a previously purchased product or service has been returned to the merchant for a refund.
- Complete the form in its entirety, attaching any pertinent documentation that will back your claim. Include all details.
- Fax or mail the form IMMEDIATELY to the Card Services Department at Apple FCU.
